Doctors Offices

This page is set up for the Doctors offices in Cleveland County.   Information on DNR forms and The Medical Necessity Forms for Ambulance transport are available. 

The DNR is a common form among EMS, Nursing Homes, Doctors Offices, Hospice and Hospitals with the same golden color and stop sign.  Cleveland County EMS will no longer supply these forms.  They can be ordered from the OEMS website. (Please see DNR  for further information.)

Medical Necessity forms for Ambulance transportation are a required form for all transports from a Doctors office  to "a destination" ie. home, hospital, or Nursing Home.   Please see Medical Necessity bar on left for the complete form.
