Section I.  Performance Characteristics

 

Outlines job performance and personal characteristics.  Evaluate the performance of the employee on those pertinent characteristics contained in this section for the period under review.  Be objective and do not let your evaluation of one factor influence your evaluation of another.  Use the lines provided to comment on the specific instances of the employee's work history, which led you to make your judgment as to his/her level of performance on that particular characteristic.  After all factors have been scored, determine the overall evaluation of the employee's performance and record the total score on the bottom of page 6.  A total score of 16 indicates standard job performance.

Exceptional Performance*

Evaluation Code: 4

Fully meets all job requirements for the position. In addition, performance consistently and significantly exceeds job requirements in important aspects of work. Work is performed at a sustained high level of proficiency. The employee also expands the scope of tasks and responsibilities or the amount of work performed resulting in increased productivity for the work unit. The employee accomplishes the most difficult and complex assignments with minimum supervision and maximum quality. Work is performed at a level that provides a role model for other employees. Specific examples of such performance are readily available.

 

Exceeds Standard

Evaluation Code: 3

Fully meets all major job requirements identified for the position. In addition, performance frequently exceeds job requirements in several important areas. Supervision is required only for special or unusual assignments or problems.

       

Meets Standard

Evaluation Code: 2

Fully meets most job requirements in a competent manner. The employee may occasionally exceed some job requirements. Accomplishes duties in a reasonable and consistent manner demonstrating full proficiency in the job. If there are occasional lapses in performance, they do not create any substantial problems for the organization, nor have any major impact on service delivery. Normal supervision is required.
