Nursing Homes

This page is set up for the nursing homes in Cleveland County.   Information on DNR forms and The Medical Necessity Forms for Ambulance transport are available.  

DNR Forms are going to change in the next few weeks according to OEMS.  It will be a common form among EMS, Nursing Homes, Doctors Offices, Hospice and Hospitals with the same golden color and stop sign.  Cleveland County EMS will no longer supply these forms.  They can be ordered from the OEMS website.  Please see DNR bar on left for further information and ordering. 

Medical Necessity forms for Ambulance transportation are a required form for all transports from Nursing Home to "a destination" ie. home, hospital, or doctors office. Please see Medical Necessity bar on left for the complete form. 

The nursing facility who called the ambulance for transport is ultimately responsible for the bill.  If Medicaid (of any type) or any other coverage wont pay, the bill needs to be directed to the nursing facility.
